In order to help us assess any improvements that can be made to the CMPC website, we ask you to please take a moment of your time to complete the following survey. The survey will be anonymous, so we urge you to please provide as honest an answer as you can to each of the questions.

Please note that the survey usually takes anywhere between ten to twenty minutes to complete. Any visitor to the site is welcome to complete the survey.
